    Minneapolis Crisis Facility Attack Leaves One Dead, Four Injured; Man Charged

    A violent attack at a Minneapolis residential mental health facility left one man dead and four other people injured, according to court records, with a 31-year-old man now facing murder and assault charges.

    Derek Duane Faulkner was charged in Hennepin County with second-degree murder and four counts of second-degree assault with a dangerous weapon following the early Monday attack at Nancy Page Crisis Residence in Minneapolis’ Loring Park neighborhood.

    Police were called to the facility shortly after 6:30 a.m. after several residents made 911 calls reporting that Faulkner was attacking people and threatening to kill them. Officers arrived and took him into custody.

    According to the criminal complaint, Faulkner told officers, “I think I killed someone” as he was being arrested.

    Investigators found one man in a room with serious injuries to his face. Court documents say the man had also been sprayed with a fire extinguisher. Another resident was reportedly attacked while sleeping and suffered a facial injury.

    The complaint describes a series of attacks inside the facility. A staff member who went to assist one of the injured residents was allegedly placed in a chokehold before Faulkner stabbed the worker several times in the arm with a fork.

    The employee managed to escape and lock himself inside an office. From there, he reportedly saw Faulkner take a log from a fireplace and go upstairs.

    Another resident told investigators that Faulkner struck him with the log and threatened to kill people. A fifth victim told police that Faulkner entered her room and struck her in the head with a lamp post.

    The man who was initially found with severe injuries later died. Court records identify him as 23-year-old Bryce Alan Hayes.

    Hayes’ family, through an attorney, said he had moved to Minneapolis with his mother in 2024. His mother later relocated, but Hayes chose to remain in the city. His family said he was only days away from transitioning to independent living when he was killed.

    The family’s attorneys are seeking answers about how Faulkner came to be housed at the same facility as people receiving mental health services.

    Nancy Page Crisis Residence is operated by People Incorporated. State records describe the facility as providing intensive residential treatment and residential crisis-stabilization services for adults experiencing mental health needs. The Minnesota Department of Human Services currently lists the facility’s license as active.

    People Incorporated has said it is cooperating with law enforcement but cannot discuss the case while the investigation remains active.

    Faulkner was already facing other criminal charges

    The Minneapolis attack came while Faulkner was involved in separate criminal proceedings.

    According to court documents, he had been civilly committed to the Nancy Page facility the previous week in connection with an attempted-murder case in Sherburne County.

    In that case, Faulkner is accused of shooting a man at his family’s machine shop in Zimmerman on Aug. 25. He also faces separate assault charges in Anoka County stemming from an incident at a hospital in Coon Rapids, according to court records.

    The Sherburne County case involves charges of second-degree attempted murder and second-degree assault with a dangerous weapon. The Anoka County case includes four counts of fourth-degree assault related to allegations involving hospital staff and a security guard.

    Faulkner remains accused in all of these cases, and the charges have not been proven in court.

    His Minneapolis case is moving forward in Hennepin County. If convicted of the murder charge, he could face decades in prison.

    The investigation into the facility attack remains ongoing.
